EPSI Online Data Entry Forms

A child’s problem solving events are counted (tallied) on the EPSI Recording Sheet. If you have an account with the online Child Data System, you can enter these counts into the system using the data entry form below. Entering these data into the data system allows you to access sophisticated progress monitoring capabilities across individual children and entire programs.

Below are samples of the online data entry forms used to enter EPSI data. These forms have been carefully constructed to mirror the paper EPSI Recording Sheet used by assessors in the field.
